A Fact Model is a static model which structures business knowledge about core business concepts and business operations.  It is sometimes called a business entity model. The fact model focuses on the core business concepts (called terms), and the logical connections between them (called facts).  The facts are typically verbs which describe how one term relates to another.

The fact model example here shows the fact model relating the teams of vehicle insurance product: the policy holder, driver, policy, company and products

This fact model example shows a basic Fact Model from which you can derive facts such as a student enrolls in a class, a class is offered by a university, a university offers a class. This simple model tells you much about this business. For one thing, students have to buy their books; books aren't provided for students as part of the class-registration process. This is an inferred business rule that's expressed as a Fact Type.
